A metà tra il legal-thriller e l'horror metafisico, il film di Derrickson ci mostra il processo dello Stato contro padre Moore, accusato di aver indotto Emily Rose alla morte: il prete l'avrebbe spinta ad abbandonare la cura medica prescrittale a rimedio di una supposta patologia psicotico-epilettica, per sottoporla ad un tentativo di esorcismo: perché di possessione del Diavolo (anzi, di sei demoni) avrebbe sofferto in realtà Emily.
